What does the red splash near my sword in the inventory mean?

It means your sword has blood on it and it affects your intimidation. To clean your sword you have to use a grind wheel.

you should be able to clean your sword of blood with a bandage or something. that would be cool feature.

For some additional payment, they can clean "another sword". ( ͡° ͜ʖ ͡°)

A bloody sword has a charisma penalty but an intimidate bonus.

Yes Blood. You can clean it on a grindstone or repair at the shop. Repair Kits don't clean. It can affect but I have Charisma at 20 and my mace is all the time bloody as hell. So, I don't really care about cleaning weapons anymore.
