I know I can do that, but I want to have a big ol sword on my back while I use a straight sword and shield and the base game doesn't allow me to do that.
you could try and send a whisper to this person on twitch to ask how they are doing this. since the position of the sword is more horizontal and its on your waist, it might look silly to have a huge ugs there, and you might be limited to straight swords, greatswords etc.
doing something like this would still allow you to play pvp and co-op online, but the reskin is client side only, so you will be the only one who sees the sword, other players will see a normal arrow/bolt quiver. doing this quiver reskin also allows you to have the sword on your back/waist without contributing to your equip load, and if you want to actually use an ugs, you can swap to one using the menu. "start>X>X", "esc>E>E" or whatever the xbox controller equivalent is, is the combo of inputs you want to press as fast as you can to open the right hand first slot weapon menu, and with a bit of practice, you can do this very fast and reliably. a possible downside here is that if you swap to an ugs to use it, the arrow quiver reskinned to look like a sword will still be on your hip.
you can start poking by around google for terms like "dark souls 3 reskin tutorial" and you could also make a post on r/eldenringmods
