Travel back to Farron Keep bonfire, climb up the ladder to Woods and explore the area further - there will be Crystal Sage boss fight, then go further to Cathedral of the Deep and beat boss there - doll will drop from the boss.

You need to go and clear the cathedral, which requires you to clear the swamp to reach, which if you havent gotten here how the hell did you get to boreal
