If you miss the well (which you 100% will in your first playthrough since you have no reason to backtrack there once you've opened the door), Patches won't appear at the bridge (since yanno, you've already done that part to get to the door) so you'll have to go the Firelink Tower route. Then if you activate the Distant Manor bonfire before speaking to him in the kitchen (which again, you 100% will on your first playthrough since the bonfire is before the kitchen), he won't be at the kitchen and will skip right to Irithyll Dungeon but the questline won't progress; if you save him he'll just stay there and won't help you during Yhorm's fight.
Then there's Anri's questline, which actually affects the ending so the fact it's ♥♥♥♥♥♥ up is pretty bad. It acts normally till the Catacombs but the only way to continue it is to go through an optional area you can only stumble on by accident on your first playthrough. If you take just a few steps past the Smouldering Lake's entrance and kill Wolnir, Anri dies and the entire questline is ♥♥♥♥♥♥.
Finally, there's Greirat's. For him to survive you need to NOT do Siegward's questline, since by the time you send him to pillage a second time you've already got Siegward's armor either by buying it or killing Patches, and the only way to save Greirat is to not get it. So you can either "save" Greirat (he still dies later on unless you abandon his questline), or "save" Siegward (he still exiles himself unless you kill him), but you can't do both unless you don't do any of their questlines.
There's just no way to play the game properly unless you read the wiki and already know what to do.
