What happens when I fuse a Lorian's Greatsword +5 and a Lothric's Holy Sword +0?

You lose weapons and are given a new weapon, Twin Prince's Greatsword. This new weapon has both weapon arts.

What happens to the upgrades? More precisely, do they get lost, or does it stay at +5?

The upgrades are lost, along with the weapons. You would have to upgrade the new weapon with new stones.

Unrelated, but curious, I seem to have a glitch were the my Lorians Greatsword shows up in the inventory normally, but when wielded, looks an acts like the Twin Princes Greatsword, weapons art and all.. I transposed it a couple patches ago, so maybe that's my problem?

I transposed the weapon prior to app ver 1.05.1 and, I haven't seen any glitches on my end.

Have you tried:
1) Leaving the weapon on the ground and picking it back up.
2) Move it to/from inventory/storage.
3) Craft a new Lorian's Greatsword.

I really do not know. I am just giving you an educated guess.

I haven't tried dropping it, but my plan was to transpose a new one around NG+++.
