This is part of an email from Bandai Nam, though they do NOT handle bans, despite people spamming them with emails, they can only tell you if you've been restricted/invalid warning OR banned/penalized.
"The Dark Souls III server team does periodically review profile/accounts that were previously restricted (to see if players have removed the data and are playing legitimately). If a player removed their data, and it did not appear during one of their periodic checks, they will most likely remove or lift the restrictions. Please note that the restrictions will be placed on the account/profile if violations of the End User License Agreement occur a second time and at that point the restrictions will no longer be lifted."
Your issue falls under the latter. Restricted which is the invalid data can get lifted, but penalized means you're perma-banned. You'll have to buy the game on a new account.
Can't be helped. Deleting the save data, reinstalling the game, nor verifying the integrity will do anything for you. Game constantly checks for data that shouldn't exist. If you picked up an item online, don't do that anymore. If you see an item on the ground, assume it's hacked and move on because they're just as dangerous as hacking. Whether you did it or not, isn't my problem, but the system is pretty good at checking it, and most people lie about not having done anything non-legit. They either got items from a hacker, hacked themselves, changed their levels and didn't math it right, gave themselves souls, or whatever else. I know people who got banned or softbanned for doing nothing, so needless to say, they aren't getting unbanned. If you're softbanned, as I said, you can get around it, but in that case, you have to not be cheating nor can you use your old file, because then you'll get softbanned again and again.

a) this must be your FIRST BAN
b) all illegal content must be removed from your account for at least 3 weeks. Apparently if you are unsure about what exact items are the problem (i.e. you picked up a bad item in a multiplayer session), customer support can tell you what you need to remove from your account. If you made a hacked character just delete it.
If you meet these two requirements you will likely be able to get unbanned. You need to contact them (bottom of the page I linked above) via email, explain your situation, and include a link to your Steam profile. They seem to be pretty hard on the one-unban-only rule, so don't get banned twice.
