I can confirm these are not patroling enemies, in fact the enemies in question are ones that usually STAND STILL in a completely idle state. Some even just stare at walls doing nothing, or sit on the ground with their heads hanging.
Further specification, I've stood at the gate to farron keep perimeter bonfire next to the black knight, and I can stand there doing nothing and then he will just walk over and slaughter you if you aren't looking directly at him. Similarly, the cross dude on the other side of the wall has actually just randomly decided to attack me while fighting the black knight a few times, when there is no line of sight between us.
Another scenario; fighting the giant crab and drawing it close to the crystal sages but still in the water, the cross-back dude sitting next to the fire in the upper courtyard has randomly agroed during such a fight, and literally ran across almost the entire map just to fight me (its pathfinding wouldn't let it hop off the small cliff into the water, it took the ramps around the bonfire instead). I know that one should ONLY agro from a distance when you are on the path above the water leading to that fire.
These events happen maybe once or twice, I can't seem to replicate the results under controlled circumstances. I'm asking if its restricted to me or if others are having similar issues.
