I lost an Estus Shard forever

I cannot get the achievement or the 15 Estus Flasks. I've revisited each and every one of the locations where the shards were and i've grabbed every single one of them, but one of them is missing, even from my inventory. Also, I picked the Ashen Estus Flask right from the start so that cannot be what's wrong.

Anyone knows a fix for this?

NG+, grab one shard. Achievement unlocked. Your estus carries over to NG+, so you just have to keep upgrading it. (I think the bonfire follows the same rule)

go to ng+

the shards respawn every cycle, so you can just get the one from undead settlement and you'll be fine.

did you accidentally trade a shard to the crow? i know she gives something for it

just go ng+ and grab the 1 in lothric wall....

@kurdtotkopf nope, maybe what I did was upgrade my estus when i had 2 shards on my inventory and the whole thing somehow glitched itself.

I had a total of 15, 13 orange 2 blue. I didn't get the achivement until I alloted all 15 orange.
