I think (could be wrong) that both games were being developed at the same time by different teams and that is why neither feels like a sequel to the other. Even though dks3 says dks2 happened before it I think what they were going for in dks2 and it makes more sense is that dark souls 2 would be taking place very far into the future after dks1.
And to be fair Im still not actualy sure if lothric is an actual place or just an amalgamation of the lord of cinders homes. It seems awfully well connected to be just an amalgamation but there are items that say things like: when the highwall of lothric appeared.... or describing carthus as a sand kingdom yet there is nothing like that in game.
