1. Guarded by a giant and leading to the main altar.
2. Guarded by a second giant. It's a big, greenish door that you can open and it looks like one of several big entrances to the cathedral.
3. The one you came through when you first entered the big hall on the base level, past a big knight with a shield that leaves exploding marks on the ground where he hits it.
4. The last one, kinda directly straight across the room from door number 3.
Door number 4 leads back to the Cleansing Chapel Bonfire. On this path there is another door, impossible to miss, that houses an elevator that brings you to the rafters. Check it and if you're still stuck in an hour or so, I'm sure we'll tell you where to go from there. ;P
edit: to the guy above me, not a bug, just usual From Software weirdness. From the wiki:
Unbreakable Patches can be found within the Cathedral of the Deep disguised as Siegward of Catarina. His spawn trigger is to open the main doors, on the left of the boss room, reload the area and then return. You MUST do this before lighting the bonfire at Rosaria's Bed Chamber.
My guess is: You yourself let him into the Cathedral after he met / robbed Siegward outside - possibly while the onion slept. Patches then slips through the door you inadvertently open for him.
Let's say bug like you said all along, convinced me.
So I did all that stuff and turns out... onion knight wasn't even over there. My damn nephew just had me convinced that he would be and I guess he should have been cause you all seem to just accept that that is where he was... but that is neither here nor there.
Anyway... I kill the Deacon and his goons and then.... nothing
There was no where to go after that. I go back to fireshrine and all these npc are there now. Including some nice sweet girl who somehow takes issue with me talking to Rosario
She says "we are adversaries now"
I'm like.. "Whuh? Do I know you? Whatever... you wanna be enemies? die." So yeah.. she's dead.
I feel like I have hit a dead end. Did I miss something? There is no place left to go now and I know full well there are at least 4 or 5 more levels in the game.
I might be kinda dumb... but I am pretty sure the game has givin me zero clues as to where to go from here.
Sweet girl is Sirris the Sunless and by equipping the Finger's Covenant or offering Pale Tongues, you essentially failed her questline.
First time through I didn't even know existed or failed more than HALF of the available side quests so yeah, it's pretty normal.
First off... you just reminded me that I never finished Life Is Strange... I should do that.
But really... after the Deacon fight.. there is no where to go except back the way you came.
Oh... Fark... wait... the second giant had some green double doors behind him... let me check that out.
So, you killed the deacons, that area is done. ( There's still much to explore, but in terms of what you HAVE to do you completed everything ). Still you are very much at the beginning of the game.
The next area to go from here would be Farron Keep as you continue your quest to find Aldrich and rid him of his soul. Remember, you thought he was in the cathedral eating men, but he left his deacons in charge while he went somewhere else. he gave his most important deacons a small doll which should by now be in your possesion.
As lore in this game tells itself very much through item descriptions, you may as well start with that doll. :)
But hey, if you want more specific directions we may also provide those, but as you can tell we don't want to spoil the experience for you - and being lost is part of it. We're not deliberately vague because we're shmocks. I mean I am, but that's not the reason for any vagueness.
