I would change "minor tweaks" for "numerous tweaks" also.
But seriously, something like this certainly wouldn't be popular enough in the vast majority of the gaming community in Dark Souls, people will get careless or mistakes will be made just because of some other reason. And seeing how even if you get super tanky, it won't keep you protected from multiple boss strikes that you fail to move out of the way quick enough. (And we all know how viable heavy armor is in here.)
But not to fear, there is a game that is sorta like what you are wanting. I can't exactly remember its name (I'll look it up later) where its Souls like, and it more or less has 1 healing item that regenerates.
I get it, when you play it very long you get very good at fighting everything - or perhaps you have superhuman reflexes. Maybe you should take a break, instead of hopeing for a change, that only a very, very minor portion of the playerbase would accept (and that only on YT, for a show off - in real life, they would throw there controller every freakin' minute at a wall).
I guess, when Demon's Souls had that feature, we never had heard of Dark Souls - because it would be never made.
Anyhoo, good luck. There was once a Mod for Dark Souls 1, where all Enemys where Hunting you right from the start - they where pulling like a magnet into you direction. That was some crazy stuff - very funny, maybe try that. ;)

Two flasks, Estus and Ashtus. 1 of every, infinite, but hard to use.
That, or make health and FP restore automatically and slowly after X seconds of not receiving damage, or something like that, so you can totally remove estus
Its not about just ME using the flask, its fine in pve but then in pvp it becomes unfair, even if you can do it. PvP is sewn throughout the game, its practically part of the enviroment, which is one of the best parts of souls games. I don't need flasks to kill things, but i also don't find it fun to gimp myself. I like to play the game how its intended.
I do however have opinions on ways it would be more fun to me. No flasks was 1 of those opinions.
That solves nothing and just creates a similiar problems like in pvp you would get people running away or hiding and flasking, only now they could do it forever. A good player can always find an opening.
Health and fop restoration have their own problems. Depending on the amount it makes everyone immortal or causes them to sit around alot waiting for it to restore.
A small FP restore would be fine, like it everyone always had a simple weapon equipped. But it would have to be the same as that so people would not want to wait for restores. That however does have its own problems such as waits to heal full. Which can be overcome by simply putting a max amount on regeneration before returning to a bonfire. This would replace flasks and be agreeable on both hp and fp.
Missed the point a bit, but the "rules" of duels are open to players debate anyway. Ive had times when i fight a phantom and suddenly the host who summoned me jumps in and ganks me cuz his friend was about to die. There are no actualy rules in the game and thats fine. WHat im talking about is a different style of play where in no part of the game do you or anyone have more than 1 flask or 0. It serves a different purpose.
While pve I see no problem.

anyway you are not forced use estus flask you know? just unequip from inventory slot
I felt estus recovery is something DS2 got right.
Just accept the game for what it is like everyone else. It's still way better than its competition. If not why are you on its forums and playing it.
