The reason you feel delay in the roll commands in all the Dark Souls games is because the roll function is also tied to the sprint function. Basically the game waits for you to release X (PS4) or B (XboxOne) before you roll otherwise it assumes you want to sprint. This is negative edge the command happens on key release.
IMO a piss poor system, but on average most people can press and release a button within 5 frames less if you try for it. So if you treat the invulnerability frames around 5 frames after you press the roll command you should be fine. Or From could give you a dedicated dodge button so you can have i0 defence on rolls like most action games.
Rolling after you get hit is part of the illogical command buffer in this game. For some reason they keep your roll command buffered for the earliest possible moment it can be performed even after getting hit. The only game/series I've ever played that does this. Most games clear your buffer when you get hit.

You really have to hit the button at the correct timing, If you spam your attack button and then spam roll, you will finish up atleast 1 more attack before rolling, then most likely roll a second time.
If not spamming button inputs maybe it is your Controller/keyboard/PC. Hell it could even be your monitor.
The queue is there so you don't spam buttons, you need to be precise with your inputs
If vsync is on, turn it off, or enable triple buffering
