Patches is not showing up

I'm at the cathedral of the deep, i opened the shortcut that is necesary and reloaded the game twice, even talked to siegwards when he's stucked in the well. What did I do wrong? Why is he not showing up?

if you visited rosaria before seeing patches then he immediately move firelink (not showing up until you enter the tower).

i'm farming those 20k souls, i hope he shows up at the tower...

you actually don't have to reload the game. I always do it same way:
get first shortcut, get second shortcut (don't open the door to rosaria, patches won't show if you open the door or ride elevator to the top, not sure which one here). Then process normally to patches, open the big door on the way to him, and he is always there. Done it at least 5 times worked every time.

He still appears if i killed the giants right? I wanted the extra dialogue too but it seems that for some people if you kill the giant he doesn't shows up...

You don't need to visit Rosaria, just walking on the rafters is enough to make Patches go away from the Cathedral; it happened to me on my second character. I'm guessing taking that elevator up (the one in the way to the second shortcut) is what makes or breaks this encounter.

He will most certainly appear at Firelink, though.

Yes, on my first character I had killed the giants and he showed up all right, with the extra dialogue and whatnot (which back then I thought was the default dialogue).

Even though it's already solved; Patches doesn't show up if you open the big shortcut door before you kill the deacons, he'll immediately move to "ambush(?)" you at the tower.

I'm pretty sure my first time through I opened every door, killed both giants, raised both walls and opened every shortcut; and he did the Onionbro armor thing. The only thing I didn't do that time was traversing the rafters.
