There's no little white soul thing. I clicked A a bunch of times (on controller) to see if it was invisible, nothing. Checked on the roof for items as well, nothing. I distinctly remember using the "Leave" option and not the "Discard" option (fooled me once with the Siegbrau on one playthrough, was pissed ), so that's ruled out. Guess the game bugged out for some reason? RIP Titanite slab...

Try going to the nest. Exiting the game through the menu entirely there. Close the entire game out. Then boot it and load the game and see if this resolves the issue. You can also try shutting down (not reboot) and then booting PC and see if it is a memory related glitch. If both of these fails you are screwed. If you have Ringed City and Ashes DLC you get quite a few slabs from those so it isn't to much of a loss.

When I first picked up DS3 I didnt know there were only 8 slabs in the base game so I went around +10 and +5'ing weps that I liked. LIKE THE STORMRULER

Except you can't, at least not in 1 (or even several) playthroughs, and they're not cheap.

Coiled sword fragment cannot be traided and can be obtained legally only once per playthrough. +10/+5 weapons are all tradable thus you can get all the maxed out weapons via trade dropping slab value to zero.

Yeah, all it requires is 1 slab, 1 weapon and 1 friend

step 1: create back-up
step 2: get weapon upgraded and give it to a friend
step 3: revert back-up
step 4: get weapon back from friend
step 5: profit
