According to Bandai support this is due to a corrupted save and the "solution" is to start a new character. However there are some people who've reported that the issue spontaneously fixed itself after they continued on with their playthrough.
I have this issue now on my NG++ run where I was trying for 100%... hopefully you are not in the same boat. I've been trying to clear bosses and progress through any other areas I hadn't already while doing PVP covenants and checking back regularly to see if anything fixes the issue. I have killed Yhorm, DS Armor, gone to Archdragon and checked back after each with no change. I've also been invaded, done some Aldrich invasions, and been summoned as a white phantom several times and killed Aldrich in other games. Haven't done any of the DLC, but my hunch is that if anything might fix this it would be related to some player interaction weirdness; seem to recall a Reddit thread where someone indicated that this was related to interacting with other players who had modded their saves.
I have never modded or anything of that sort, but I have had a hard crash (ravenous crystal lizard janked out at Farron). I haven't given up just yet, but it is starting to feel pretty hopeless. If anyone has any ideas for a possible solution please share! If anything turns up I'll be sure to come back and share.
So my bonfire reappeared and I was able to beat Aldrich and proceed into the Grand Archives!
Unfortunately I cannot pin down exactly what the trigger was, but I had cleared every other boss and area in the regular game and checked back after every major 'event', and after PVP interactions, with never any change. So I decided to go ahead with the DLC content and what seems to have fixed the issue was progressing to the Dreg Heap. Just a wild guess but maybe there is something that gets "re-written" in the game data when the Ringed City DLC locations get loaded? It's still possible that it had something to do with the phantom I summoned for Friede, but seems unlikely at this point.
Curious if someone else with this issue can try proceeding through the Ariandel DLC, killing Friede and then loading the Dreg Heap and seeing if their Anor Londo map gets fixed? Would be awesome if we could confirm this as a fix.
In any case I can now definitively say that this issue IS correctable! Praise the Sun!
