How am I supposed to save Greirat on his second outing?
First playthrough he died due to my ignorance. Now I'm on NG+ and he died even though I paid close attention to the wiki. I made sure I had talked to Siegward in the kitchen and sent Greirat on his outing before killing Pontiff. His ashes showed up in the sewers below the ktichen anyways.

Greirat will return or die after you kill any boss after sending him to Irithyll. Greatwood and Demon King are optional bosses so you can just choose to not kill them before that point.
I always kill the Greatwood before moving on from the Undead Settlement. I guess next playthrough I'll just talk to Siegward in the kitchen before sending Greirat -_-

Or maybe the wiki doesn't mention that you HAVE to talk to Siegward in the kitchen before sending Greirat? Seriously, the wiki is helpful and accurate mostly, but it's not perfect. Here's a copy/paste of what it says regarding the second outing:

Send Greirat on his second outing - Before killing Pontiff Sulyvahn, after Greirat has returned from his first outing, which is giving Greirat Loretta's bone.

^ Nowhere does it say that I MUST talk to Siegward in Irithyll before sending Greirat. So yeah, the fault lies with the wiki, and the game itself for being so darn picky.

You must:
Send Greirat off before the Pontiff
Find Patches in the Cathedral of the Deep
Either buy the Catarina set from him or explore the Firelink Shrine’s Tower (tower key from handmaid). I suggest just doing Siegward’s line.
Patches will be a vendor after you fall for his trap in the Cathedral. You can buy the set there. He moves to Firelink after you meet him again in the tower.
Buy the Catarina set and find the well near the Cathedral. Go to the Cleansing Chapel bonfire and turn right after going through it’s main doors. There are some hollows praying. Interact with the well, Siegward is stuck in the well. This only happens when you buy the entire set.
Throw down the armor. You can get it back later. Siegward will then climb out by himself later.
Find him in the kitchen in Irithyll. I think you’ve found that already. He’s sleeping, interact with him multiple times to wake him up.
Siegward is now in place. He will save Greirat.
Kill Pontiff to finish Greirat’s scavenge.
Greirat should be back at Firelink, Siegward will move to the Profaned Capital.

Note: It’s impossible to save Greirat on his third outing unless you don’t send him at all.

You don't need to talk to him. I always do it by lighting the second bonfire in Irithyll, sending Greirat to pillage and killing some boss I left behind. With that method you have 0% chance to trigger the unknown fail conditions as they are all in Irithyll where you won't go until Greirat is back in Firelink Shrine.

And what does the wiki says right after that? Get Siegward in Place - 1. Before the Pontiff Sulyvahn boss fight, even after lighting the Church of Yorshka and Distant Manor bonfires. 2. Which must be after speaking to Siegward at the fireplace just above the sewers in Irythll

-make sure you reached the rosaria's chamber bonfire
-siegward will appear in the well
-patches will appear in firelink shrine after you climbed the tower
-reach the central irythill bonfire (first one after the bridge and barrier)
-send greirat
-reload the game
-talk to patches and when he asks where greirat has gone, tell him(he will save greirat)
-beat pontiff sulyvahn
-greirat and patches will appear back in firelink shrine

Did you drink the soup found in the kitchen?

I read from wiki it might be required to leave alone for Greirat to survive, even after meeting other conditions, although it seems to be unconfirmed.

I haven´t used it in long time and had no problems with Greirat.

This is what I did. Still found his ashes below the kitchen.

I did drink the soup. If that actually impacts the questline, then that's stupid too.

I did drink the soup.

No idea if it´s the cause of your trouble.

But that is atleast one thing you have done differently than me.

But as you clear the area, you'll obviously pass by the room and make sure he's there.

Once you kill the Pontiff, simply return to Firelink and Greirat should be there.
Congrats, now send him to Lothric Castle, where his death is 100% guaranteed unavoidable, no matter what anyone says, if they say you can save him from Lothric, they're a ♥♥♥♥♥♥♥ liar.

If you kill Old Demon King and want to follow Greirat's questline, then it may fail for some a reason (killing a boss, they say). But it doesn't matter really if you kill him during Greirait's outing. But this still may be tricky and some dependencies may be abused here. If you kill Alva the Spurned the quest is over (for some a reason).

Logically, if you send Greirat first then there will be no Siegward nearby to help the thief. And if you go to the dungeon then the onion knight will follow in that direction.

I still was able to rescue Greirat with defeating Curse-Rotted Greatwood, ignoring Patches, giving Siegward Catarina set from the previous NG cycle, litting distant manor bonfire, ignoring estus soup. Yet, I used Firelink tower key (because Patches, that don't show up anyway).

I have talked to Siegward in the kitchen and still had no problems with Greirat so this should not be the problem.
