How to actually save Greirat in Irithyll?
I'm talking about the part where he raids Irithyll.
On 5 playthroughs I followed all sorts of different guides on how to save him but not once did he survive.

Once the boss is dead, return to Firelink. Greirat should be back safe and sound from his Irithyll pillage, and will mention being saved by a “peculiar onion Knight”

You either have to do the Onion Bro quest and save him from the well at Cleansing Chapel or have Patches in Firelink Shrine with Onion Bro's armour still for sale. If you didn't rescue the Onion, have the armour on you with Patches in the Shrine (or not the in the Shrine) Greirat will die in Irithyll.
I did that on my last playthrough and Greirat still died. Even followed up with Onionbro in the cell in the profaned capital chapel area.

You have to do either of the above before you send Greirat to Irithyll. There's no way the quest will fail if you for example rescue the Onion Bro from the well before sending Greirat to pillage.
Oh well, this time I ♥♥♥♥♥♥ up by crossing the rafters before getting tricked by patches.
Is there still any way to save Greirat if I send him to Irithyll now? Patches won't spawn anywhere and Siegward is trapped in the well.

talk to Onion Bro at irithyll, go back to shrine send greirat to pillage, kill pontiff and greirat should be back. Do not go to the dungeon before pontiff. and make sure to exhaust all of their dialogues. the tower key from handmaiden will spawn patches at the shrine also.

he dies for good in lothric castle but he will come back from irithyll if you save onion bro first and talk to him in irithyll. if you go to irithyll dungeon before killing pontiff it ♥♥♥♥♥ it. 🙂🙂

Usually I buy Onion armour from Patches when doing the Cathedral of the Deep. Then I save the Onion from the well. After that I progress on, Abyss Watchers, Wolnir, then cross the bridge and light the bonfire. After that I send Greirat to pillage and get Patches into the Shrine. Next I clean out Smouldering Lake. Once the Old Demon King is dead, Greirat is back at the Shrine.
SO, any way to fix it? I haven't done Pontiff or the dungeon yet, or sent Greirat to pillage. But Patches isn't spawning in the Cathedral and Siegward is in the well.

Done the same in all my game cycles, can approve n recommend that way.

This worked for me! I had to do it another way though, because Patches didn't spawn in the Cathedral because I already lit Rosaria's bonfire before meeting him.
So I bought the Firelink Tower key, encountered him there, then spoke to him at the Shrine and bought Siegward's armor off of him, helped Siegward in the well and returned to send Greirat to Irithyll.
Then I went to kill the Old Demon King and he's actually made it back.

Didn’t realize that you had already lit Rosarias bonfire, guess I should have read more of the thread, but I’m glad you were able to adapt and make it work! That’s the most reliable and least complicated way to save Greirat
