I play Shadow of the Tomb Raider at 1366x768 and everything else maxed:

And I still have some moments of lower fps at DS3 sometimes.
(but yeah, on SSD game would load areas and render "a bit" faster).
I havent used and hdd with this game so take this with a grain of salt. But. ..
This may be the issue, especially if combined with a bad connection. There's a lot of back up data being constantly overwritten each time you perform an action in game, both on the servers and harddrive. Each time you see the flame in the right hand corner of the screen...something is being saved.
Not sure just how big a diff it would be from an hdd to ssd, but based o experience from playing other games on hdd (havent had a mechanical drive in my system since before I started ds3), it wouldnt shock me if some lag was from saving.
Well there goes that theory lol
@OP reinstall DS3 on HDD and see what happens.
I have it on ssd, and I have no struggles. I keep my ssd under 75 percent capacity though, as is recommended.
