Have you checked the settings in the dark souls 3 menu ? They could have all been set to 0.
The only other problem I've experienced is after an update to my pc the speakers wouldn't work at all until I used a restore point to undo the changes.
Basically what I'm trying to say is. Does sound still work outside of DS3 ?
Same with quad, stereo, anything. Change it to how it's supposed to be, and if that doesn't work, change it to a better version you don't have.
Go to playback devices, right click speakers (the ones you use), and click configure.
this should've been one of the first ideas to try, i think we all fail here today
