I've just been doing the same, living with it, lol. It's annoying, but doesn't often get me killed or anything.
As for causes, I can think of a few. Enemy hitboxes interacting with the camera, for example. Weird collision with the geometry above the player, since the camera can definitely collide with solid objects, such as walls. It's possible that the hitboxes at the altitude of the camera are less polished and more prone to bumping the camera.
Nope, even double checked to make sure my controller or anything else wasn't plugged in.
I'm not sure if there's any way around it. IIRC, there was some way to break out of the spin (if you just try to compensate for it, you have to "fight" with the camera for a bit until it stops), but I don't remember what it was (maybe if you use the reset camera/lock-on it snaps out of it).
It's just something you'll need to deal with.
New to Dark Souls. Every time I fight an enemy the camera doesn't really stand behind me.
It just goes wherever. Is this normal?
