For pure bleed you can go Katana, Carthus Curved.
I just found the Brigand Daggers. Are twin blades not that good?
2 Poison is crap.
3 Start with thief class for a dager and a bow as starter kit. (the thief dagger has bleed)
4 Poison is crap.
5 Armor depends on your playstyle, but agile and fast assasin needs no shield either. That means you should keep the total wheight of your armor less than 30% (gives you fast and long roll).
6 That means you have a "quality" build, but you should try and get 40 dex and 40 str. You need to upgrade your weapon with a refined gem when yor level is getting high.
If you invest in luck you should go down the hollowed path, but its kind of tricky though. (I recommend skipping the luck and favor the health and stamina).
Poison is situational. great for pvp. great for some bosses. good on most pve enemies unless you are in the hellish area or swamp area where they live and breathe poison.
Bleed is situational. decent for pvp. great for some bosses. amazing on some pve enemies.
Just use common sense when it comes to the statuses. If it looks heavily armored or tough scales try poison first. If it looks soft and fleshy, gut it like a pig and bleeeeed. Eventually find a frost weapon and you can freeze them first before applying others to make them attack/dodge less.

3. There are several weapons with innate bleed that are really good when infused. You should try them and choose the one you prefer. You can get the Bandit Knife quite early from Greirat. The Warden Twinblades are also popular for bleed. Personally, I love the Carthus Curved Greatsword.
4. There are not as many weapons for poison. I really like the Rotten Ghru Dagger. Also, you can infuse it with bleed to inflict both effects at the same time (it's surprisingly good).
5. Armor doesn't matter that much, choose what you like.
6. If you want to focus only on bleed/poison, go for 40 LCK (50 tops) and the minimun requirements for the weapons you want to use (don't forget to infuse them!).
I have a DEX/LCK character and it's a blast.
Put poison gem when you can into the Guru rot dagger
Theres a staff which has a poison mist as a weaponart which is really effective. Considering that you d need a bit of Int for the spells anyway, go for it.
