I had this happen as well on my laptop. My laptop has Intel Integrated Graphics as well as a Nvidia GTX 965M. What I had to do was force the game to initially run with the integrated graphics. I set the game to run at my desired resolution and to run at full screen. I then exited the game, and used the Nvidia control panel to set my default graphics back to my Nvidia card. Then, when I ran the game, it came up properly.

I just had this happen to me since I'm also on a laptop:
1) Right-click on your desktop, then open NVIDIA Control Panel
2) Head to "Manage 3D Settings" > Program Settings > Select Dark Souls (darksouls.exe), or add it if it isn't there.
3) Select the preferred graphics processor for the program: "Integrated Graphics"
Then the game launches properly.
4) In the menu PC Settings, change it to Fullscreen and select your resolution
5) Quit the game
Open NVIDIA Control Panel again and this time select High-performance NVIDIA Processor and you're done.

Another poster writes about PS4 controller problems after the first fix didn't work for him:
Not working here. It wont load in integrated either. What I did iscover however is that it only Stops working If im using a DS4 controller. this just happened to me today after the newest windows update.

Did you, just for the lols, unplug your controller and tried starting the game once more? Seeing as the first two fixes I posted speak of a laptop, maybe the third fix will be the charm.
Other than that, there were lots of hits for 'dark souls white screen', so I'm sure the solution is anywhere out there.

Update: Never mind.... Stops working after a while....
I stopped playing the game after 20 hours of play, it crashes all the time, I already reinstalled the game and checked the integrity of the files, but nothing helps, what else can I do to solve this problem? Please help me, I'm an ardent fan of Dark souls series and not I wish that would all be so sad! Help me please))))
