Sword on table/fog wall but can't do anything
In the ringed city dlc some phantoms just led me through a hole in the elevator wall down a ladder to a room with a sword on a table and a fog wall in front of it. I'm assuming there's meant to be some sort of prompt to Examine. But I can't do anything in this room. What's wrong?

If you started a questline (Shira asking you to kill the abyssal dragon), then you can activate a bossfight here. Seems like you missed it.

EDIT: You may be able to activate it still. The big staircase with 3+3 big Herald knights on it, go to the top of the stairs, there will be a closed door, talk to the lady, and accept her request.

I dunno if you met the dragon on the bridge, if not, you have to defeat him there too (resting place or what the bonfire is called).

Oh lol. I did talk to her but my dialogue got interrupted by some fat giants walking up the stairs towards me

He just has the knock off the dragon to do it you can ignore the rest

Kill them first. And you need to talk a LOT to her. You can jump attack the ♥♥♥♥ out of those fatties.

Is it even possible to ignore that big ass dragon in the middle of the bridge you have to cross?

well you have to knock it out/off the bridge

Unless kill dragon at first place there will be fog wall always ...

Like others have stated, the lady behind the door tells you about a dragon, then you encounter him twice. You have to defeat him to make him go to that fog door area.

Will he return to that bridge where you get the chance to melee attack him. I was attacking him there but then he moved back and I ended up running offthe edge to my death.

If you haven't defeated him he will reappear, just go to shared graves bonfire and run to the bridge.
