3 Nightmare grenade = tormented soul + canister
1 Oil flask = oil barrel or cup of oil or bottle of oil + empty flask

Actually, the link to the crafting guide is a tad more useful, it gives those posting on grenades the opportunity to see many of the other possible things they can craft as they scroll through it. Like arrows and scrolls and armor and potions and the Sword of Planets, etc., etc. I used Tolben's guide extensively during my first playthrough, it pretty much made the game a cakewalk, aside from figuring out the puzzles. But there's a lot of files associated to the game through Steam these days providing information for players at differing skill levels, so it can be a bit of a hassle to winnow through those that are genuinely useful for those starting the game vs. those that are in another language, or just apply to achievements, etc.
