How do you play local co-op?
I went through all of the menu's and the second player/controller never had an option to play!

You both have to use a controler. You cannot have one on keyboard/mouse.

I had two controllers on, but when you start a multiplayer game showing the two avatars, there is no option for a second character to join or pick. When you enter the game, the second controller doesn't do anything either.
You have to launch in single player then have second player press start on controller 2.

Multiplayer is not for local play. You'll notice on the main menu when you hover over the Single Player option, it lists that as the choice for split screen. Why they organized it that way is beyond me :calm_creep:

I figured it out! Before launching the game, in the main menu, you press start on the second controller, then press X to create a new profile. Type a name in, newGuy press A to accept, then the main menu shows a +newGuy under the main profile! Voila! Local multiplayer, local co-op!

Is the local co-op (as in a single PC) possible with more than 2 players?

No, split screen co-op is 2 player only (or 2 controllers, anyway; you can always pass controllers around, but the non-combat sections wouldn't be quite as interesting for the odd people out).
Online/LAN co-op is also only 2 player officially, but there are mods to allow 4 players.

1. Need two controllers
2. Turn on Controller mode in Configuration > Game
3. Now second guy press start button
