Don´t before level 21 (final level) - you´ll find exactly one (that is different to DOS Classic, where you could reliably find 3 at least, I found up to five). It is simply too valuable to waste at low character levels (in DOS EE you´ll find the one and only normally pretty early - around lvl 7 if my memory serves me)..
I think its because the devs don't want anyone to have fun in this game, its suppose to be like work.
Also, I did not take out the townsfolks, except for the guy who refused to let me into the Cyseal prison's armory even after I saved his life. Maybe people who did take out all the townsfolks would reach level 23.
They always have to find a compromise between the people who like it to drop more than once and the ones who whine about uniques that drop more than once.
So it's less about the devs not wanting anyone to have fun but about the devs trying to minimize player complaints (that will come no matter what they do ...).
Some stuff from Classic was changed in the Enhanced Edition due to player whining during Classic.
