In front of the skull blocking access to the pirate ship and the boss here. Have pressed all the switches but cannot move the giant skull.

The switch you need to use is the 'Blood-Stained Switch'. It's not hidden, but isn't in an obvious place, either. It is down the stairs to the south east of the skull and then to the west, past a plant that looks like it might block passage, but does not.
