Bizarre XBox 360 controller problem
My XBox 360 wired controller has been degrading for a while so when I first encountered the problem I'm about to describe, my initial response was to fix the wire on the controller; I can now confirm that it works perfectly and the controller hardware is not responsible for this issue.

The issue is that my controller disconnects during battles when I hit an enemy with a standard attack, every time, without fail. I am stuck on the first battle of the game, and it happens the same exact way every single time: I can run around town, go through menus, and do everything else perfectly and then the minute I get in the fight at the town gate and strike the zombie enemy my controller disconnects as soon as I get my first combo in. What the heck is going on? I have never, ever heard of a glitch like this in all my years of gaming.

I just tried validating game files and the process said that one file came back invalid so I am currently reinstalling, but damned if this is not the absolute strangest consequence of a corrupted file I have ever seen; and that's assuming it fixes the issue. I'll report back when it's done

EDIT: It didn't fix the issue. Anyone got any ideas?

Fixed. The issue had something to do with controller rumble emulation shorting out the controller's connection, and it stopped when I turned Rumble off in the game's settings. This fix ought to be pinned because I suspect it's not uncommon.
