Kingsoul takes 5 slots. Void Heart takes 0 slots. Did you use any mods at some point? (they interact with your save files)

i used a mod to get void soul since king sould didn't change into void heart when i dream nailed my reflection and i went through the cutscene

Yes, using mods can sometimes lead to these kind of glitches in the game with something not triggering. Last topic with a similar problem was about Grimm.

If you don't want to start playing again from your last clean save backup (before any modding), then you have to edit your save file. Backup first.
