Soul gauge won't fill up all the way

I'm 84% done with the game and all of a sudden I'm having a problem with the soul vessel gauge not filling up all the way.

What I mean is, when I collect souls it fills up my main meter up to 70%, then it starts filling up the spare ones instead. So now I'm only able to heal 2 health points at a time instead of 3. Very annoying and seems like it's going to make the rest of my playthrough much more challenging.

On a side note, I'm also having a problem with the quick map (LB). It keeps opening up the big map like I'im viewing it from the inventory. If i exit and press it again it will be normal, but happens very frequently now.

EDIT: I'm not exactly sure, but I think this started happening when I failed to do the Flower Delivery quest, and then exited to spawn back at the start of the quest so I could try again.

Did you die for the first time? Do you notice the Soul Vessel is... cracked?

That's a feature, not a bug. You'll have to destroy your Shade to recover your full power. A Shade will be produced at the point you died, or near to it. It will be marked on your map. Otherwise, you can talk to Jiji at Dirtmouth and he will instantly retrieve your Shade, at a price.

Also, if you press the button for the quick map twice in rapid succession, it will open the full map as if from your inventory. Perhaps you are pressing it twice, or the button might be a bit trigger happy itself.

Thank you, I just notice it was cracked. I never died much in this game so didn't have this problem before. I noticed my shade is in the arena section. Didn't have a lot of souls so wasn't concerned about retrieving it. But now I know it's not a bug.
