Missing mask shard

So IIRC you start with 5 masks/hearts
I currently have 8 + 3 mask fragments, meaning there is a minimum of 16 mask shards to be found.
However the Complete map 1.3 http://steamcommunity.com/sharedfiles/filedetails/?id=876122772 only lists 12 with this guide listing 13 http://steamcommunity.com/sharedfiles/filedetails/?id=874455821

I am fairly certain I have all of those. Im a little uncertain about the one in deepnest since there is no pedestal and I dont remember getting it, and I dont remember if I got the one from the NPC that upgrades the Dream Nail.

So there are 3 more potential mask shards one of which I am missing and its starting to drive me crazy

For the Maps of Hollownest, the 5 mask shards that aren't listed in the text (three are on the maps) are the following:

1) Sly sells a 4th Mask Shard for 1500 Geo (requires Storeroom Key)
2) Stone Sanctuary (Greenpath)
3) Forgotten Crossroads - Room directly below Hot Spring (requires Double Jump to access)
4) Hive (Kingdom's Edge)
5) Queen's Station (Fungal Wastes)

I actually got the one in Forgotten Crossroads without double jump XD
I died there and used my "ghost" to down-attack him in the air and that gave me enough to reach the ledge...
