One mask shard left, need help ! :D

Hello,
Well, i still didn't go for the last boss, i want to have 100% before.
I'm missing one mask shard. I have the one with the flower quest, the one with the fountain and the one after i have save the lady bug. Any idea where he could be ?

Also, i have meet a NPC who is a funny mushroom 2 time. One time in fungal waste and one time in kingdom ledge (you need the mushroom charm to be able to talk to him). Some one has meet him a third time ?

And, there is a strange wyrm who block a way in kingdom's edge. Anyone know how we can convince him to let us go throught ?
