It's a little bit of both. Mostly bad writing.
The Duke probably can't make a scene in front of all those people around.
Technically you're meant to escape through the secret exit in the jail but the game developers decided to straight up not make the guards throw you back in jail if they caught you so there's no reason at all to be stealthy about your escape and you can take whichever exit you want as a result.
The map marker is buggered on either the main Map or the Mini-map during the escape portion, it directs you to an unreachable location.
I don't think he remembers strangling her. He was drooling and going crazy and all. Part of a bigger story since he was having a flashback with his own encounter with the dragon (like how Feste notes how he was walking toward the chamber), but it doesn't give a good impression since the usual interaction with the NPCs involved don't change after the encounter.
Would've at least helped if Aelinore mentioned the weirdness.
Other dialogue options (with other potential romance partners) are more limited for male characters.
Are there even anything for female characters (straight ones)?
Of course, people have been getting Fournival and Caxton as their Beloved without even trying...
