Supplier's Demand never happened

I talked to the Duke, I did Idol Worship, but Madeleine never had a quest for me. I didn't escort her at the start of the game. What's the deal?

What about the quest "Chasing Shadows"? From Mason, the guy who greeted you at the capital.

I failed that quest. Apparently I got too close.

Not like a physical shop. She still sells stuff though.

Well, the quest is for the shop's inventory not her backpack ;)
If she's still a travelling peddler her quest line progress route (escort, Chasing shadows, Supplier's Demand, Bad Business) has been blocked.

Yep, you r screwed as far a Madelin goes since you did not escort her at the beginning of the game or complete chasing shadows. VERY hard to loose the guy you are following in Chasing Shadows so it's ok to stay well back. When he goes around corners a cut scene will hit when you follow and when it ends you are fine to just walk in the direction he went in the cut scene.
Generally he give you one warning about being too close and after that you r toast (as you know). Talk to the vendor that sells armor in GS. Perhaps he will still accept/ask for a gold idol. You've still got NG+ to compete quests you either failed or did not attempt.
