It's entirely likely that you are hitting one of the various Arisen's hearts. However, given the resonance shown during the cutscene immediately after, you may be damaging a representation of your own heart, maybe simply removing shielding from it allowing that resonance, which seems to cause a severe twinge to both Grigori and the Arisen, which results in the fall onto that old battlefield.
