You just need to focus on using your long jumps to dodge attacks and attack the head when it's clear to do so. If the boss summons healing crystals, you need to destroy them with explosives as soon as possible.
Eventually Nihilanth will enclose the arena and start increasing its attacks. Utilize cover (the boss can destroy it, so be aware), use long jumps to dodge attacks, and during the whole battle, Black Mesa scientists will send you care packages and some extra caches will also be available. There will be four spots you can heal and rearm at, but once you've used the four spots up, that's it.
Keep unloading everything you can at the head until it splits open and then get your final shot in!
1, destroying the three crystals
2, bringing down the shield and damaging the boss

4, the boss getting mad, shrinking the arena, and tossing out waves of attacks

Also, if things get too bad, you could just start the Nihilanth chapter from the main menu maybe? You probably unlocked it by starting the fight.

I've even been able to beat it without taking any damage, even though that was pure luck as my previous recording was so bad that I restarted the level just to produce this outcome.
