This appears to be an issue with Windows 11 24H2.

Yea this isn't an issue in Windows 10.
Go to the root folder of the game and right-click on the game. Click to the properties and find the item "full-screen optimization" or something like that. Put a check mark there, and after that everything should work.

Easy quick fix:
1- Launch game with the launch option. The changes below do not work from this screens
settings option.
2- Let the game load in windowed mode.
3- Start the game play while in windowed mode.
4- Pause game and select options.
5- Select display.
6- Turn off VSYNC
7- Turn on fullscreen.
8- Turn VSYNC back on.
You should be good to go from there.
* This has to be done every time you launch the game until a patch is released.

Confirmed this worked for me. Glad there are people like this in the community helping out, it's a shame 2K and Hangar 13 are still letting people buy this game, even enticing them with sales knowing how broken it is. Guess that is the kind of goodwill that is going to make people rush out to buy The Old Country
