Speaking of which I really wish there were a way to make the frame rate go above 60. Despite the Options: Display "unlimited" setting in the game mine won't do it. I tried other posters' solution of "Disable fullscreen optimizations" in the game exe's Properties but that did nothing.
Thank you for the reply. Tomorrow I will try, hopefully it will work. The weird thing is that for the first couple of minutes the game is smooth but then it starts to decrease the FPS.

There is a similar issue, not exactly the same, with Atlas Fallen, which is also fixed with Lossless Scaling. Unlocked framerate feels terrible even at 150fps+ - locked to 60 feels better, but not great. Lossless Scaling makes locked 60 feel even better at 120fps.
When I set to window mode I can reach 115FPS but then it drops again to 25.
I do not understand why because after 4 weeks of gameplay it was fine and now it started. I updated everything but still!
Make sure your GPU heat sinks are clear of dust and have free airflow around them. And that the GPU fans are all working, and the case fans are also working. I have seen fans go bad and stop working.
If could also be background processes running that kills your FPS. Did you install any new software from when the game worked fine?
Such as using the extremely inefficient 2K Game Store launcher, it can replicate itself several times in your computer memory which bricks some computers. The 2K launcher is easily bypassed.

There could be many reasons.
I ran the fans test with MSI Center and they are working perfectly fine. Now I add Middle Earth and Mafia 3 on the gaming mode of MSI center and now both game are working perfectly. I played it for 20 minutes each. Now I don’t know how I sorted but I think that probably the fans went off with those two games so that’s why after a couple of minutes the FPS dropped drastically.
I'm literally pulling 90+ fps at 7680x2160... but it stutters? I have tried changing everything to low, resolution minimal, fullscreen, windowed, disabling fullscreen optimization on the exe... just don't even know anymore. I played one mission, it's barely playable and so not worth the bother if it's going to run like this.
I remember it running like ♥♥♥♥ 4 years ago but it should have changed!

You have several options. Try Borderless Gaming to make the game smooth and be running at what you would expect 90fps to look like.
Run the game, make the game windowed, alt-tab out, load BG, add the executable to the right hand section of the app, then hit the third button from the top, go back to game
or
Use Lossless Scaling with Framegeneration (makes borderless or windowed go fullscreen) and turn that 90fps into 240fps at least. (while fixing the problem)
Not sure why this happens, but i find Mafia Def Edition did the same thing and Atlas Fallen also has a similar problem (only LSFG fixes it as the devs are ignorant dolts)
When I installed my OS on one M.2 and the game on another M.2 all the stutters stopped for me. So I can only guess it is the high (and weird) resolution that is the problem since this is a console port meaning it is made for smaller, normal resolutions below 60 FPS.
In fact some parts of the game you will get stuck at if the FPS is over 60 FPS because of the way the game was coded to use the FPS to determine game play.
Would that lossless scaling be application specific settings of the nvidia app?
I'll usually do frame generation but didn't know that's what the lossless meant!
