2. If each underboss has 3 districts at the end, the tie breaker is whoever got his or her 3rd district first.
Your post suggests that Cassandra already got her 3rd district. If this is indeed the case, you have to give the French Ward to Vito as well so that he ends up with 4 districts. Whether this will cause Burke to revolt depends on your earlier actions, in particular when you gave him his 2nd district and whether you gave him rackets and took them away again. You can also increase his loyalty by doing his side missions.
The wiki says that you have to give an underboss 3 districts to unlock all of their agenda missions. I got all of Cassandra's missions when I only gave her 2 districts, though. No idea what factors into this.
Vito does not automatically get the 'ending' doing that. First time I did the above and Burke got the ending.
I figured it was which underboss has the most max earn, because some districts earn more than others. So even though they had equal number of districts, they did not have equal max earn.
So in my game, Vito got it because despite giving rackets to burke and cass, I always gave the district to vito in the end, meaning he's still making the most, cass and burke were literally left to their original starting district, in my case
from everything I've read, if you do ALL of their side missions before you start doling out districts, you'll gain enough trust from them to not turn
As for the underbosses sub-boss missions, you can only do them if the underboss has a racket in a district that is not at max earn.
I mean besides reading, I've actually done it so it's safe to say I have firsthand experience, maybe its an oversight with the weird way I played out the game or maybe it's intentional. I don't know but I've done it already and that's all I can really add to that.
