Only the right joystick to move mouse or look around is functional with PS4 controller connected to PC. Steam overlay and big picture mode both off.

i can confirm that i am also having same issue with my xbox one controller
Try Enable steam overlay or restart computer.
Has someone fixed it? Having the same issue with ds4
same issue, tried a lot of so called fixes and still nothing, even tried an xbox controller and still nothing
Try this guys, should get full control, the prompts (the game only has Xbox), and rumble.
This'll be for Windows - don't know what things are like on Mac. Don't disable the overlay.

Enable Steam Input for Mafia III:
Right-click M3 in your Steam Library > Properties > Controller > select Enable Steam Input

Apply my config, modified from the official/default config. I added LS Click to Left Stick's Click Action on the Menu Controls action layer so can toggle Wiretap but also means Xinput will be used (rrrruuuummmble!!!).

1. copy this: steam://controllerconfig/360430/2819419724
2. Press Windows key + R, paste the copied link, OK.
3. Should be seeing Steam's Controller Configurator. Choose Apply Configuration, Done.

Not seeing apply in Controller Configurator. Is the game supposed to be running?
When it is displayed, 'Apply Configuration' would be next to the Back button at the bottom of the screen, it doesn't require the game to be running.

Did you have the controller connected? I don't get the Apply Config option when my controller is not connected. If it was connected, move the mouse about in the configurator and see if the option is displayed when it switches to mouse UI.

If it's still not displayed, check things out at Steam > Settings > Controller > General Controller Settings. Is the controller listed under Detected Controllers?
same issue with xbox elite 2 controller and is super broken an annoyed.

Disable Steam Input in the global setting and per-game setting
global: Steam > Settings > Controller > General Controller Settings > disable Xbox Configuration Support

per-game: Right-click the game in your Steam Library > Properties > Controller > select Disable Steam Input.

or try the method I posted in post #5 if you want to be able to use Steam's controller configuration tools
