This game, I assume purposefully, makes it harder to distinguish enemies from their surroundings. Tinkering with your brightness settings can help a bit (and definitely turn off bloom).
You're *meant* to be moving slowly and carefully, and not running in circles firing your gun on full auto (even though you definitely can do that effectively if you're lucky and/or your enemies are bad).
The best advice I can give you in terms of how you're playing is this:
Try to only check, and be exposed to, small portions of a room at a time. Don't walk through a doorway, look left and right, and expect to get the drop on anyone. Use angles to check portions of a room, one at a time. It helps you focus and spot people much easier.
