How does the cooldown system work?

Can someone explain me how does the cooldown system work? I got a 2 hour ban ( which I really deserved xd I'm not complaining just wanting to learn.) I want to know how the cooldown system works. Here are my questions;

Do we get a cooldown if we get kicked?

Do we get a cooldown if we kick too many teammates? (Like CS:GO)

How much team damage do we need to inflict to get kicked/banned? (Or how many teammates we need to kill to get a cooldown)

Do the cooldown level decrease over time? (Like if I don't get banned for a week my next cooldown will be 15 minutes.)

Do we get a cooldown if I inflict too much team damage but don't kill anyone?

How many times can we kill the hostage without getting a cooldown?

I've just started playing ranked and got my rank so I have lots of questions :P By the way any extra info would be appreciated and don't mind the grammar and spelling errors :P Thank you for reading.
