In real life there have been cases of people surviving headshots.

This should not be surprising, as there is always that "1%" in play.

Game-wise? No.

If it's a t box shot (draw a T shape around your eyes and nose) then you are dead instantly since the bullet will destroy the medula oblongata.

Most types of headshots are lethal but not always instant.

In the game, even a grazing scalp shot is lethal. The only weapon that doesn't OHK to the head is Caveira's magic pistol.
