How do placement matches work?

Now i've been putting quite a few hours into Siege and finally feel comfortable approaching the big boy ranked mode. I am unsure about the placement matches though and hope to get some clarification here.

Firstly, are these matches a one time thing or are there new placement matches each season?

Secondly, what is actually taken into consideration to estimate player skill? Is it just personal performance, team win rate or a combination of both?

Last Season MMR +/- Amount of Won/Lost Games = Rank
You will tend to place lower than your last season, most games do that.

You lose your rank at the start of each season, and both k/d and w/l are taken into consideration

Nah you'll start at 2500 MMR regardless of last seasons finish. You can check on external stats sites what exact score you have during the placements.

Each season. Rank reset onto 2500 MMR.

Only end results - win or loss - count. The amount of points adjusted depends on the balance between teams and the uncertainty factor.
