I might be remembering the wrong one, but you need to go into a cave entrance that's west of the fort and then it leads to a gap you have to jump over. The cave entrance is hidden because you have to actually climb up a short cliff to get to the area where it is.

Specifically, when standing outside the fort and facing the fort, climb up the cliff to your left. Once up top there should be enemies. Deal with them and you may need to continue a little bit but eventually you're reach a spot with a campfire and a nearby tent. The short cliff is near there.

you can climb to it as titan said (a bit tricky to find), or just fly a drake up there. gg ez

its easy to miss because you can actually climb the frozen water fall.

I got the one at the top of the frozen waterfall already I was talking about the other one.

Hover your mouse/controller/whatever over the memory icon on the map and it should show you an outline of the different paths surrounding it.

Explore where they begin, you're bound to find an entrance.

If it's the one that's near the fort, start from the fort and climb the nearby cliff to your right. Look around that area and you should find an entrance to a cave

I finished the obnoxious Carnan missions so I could fly a drake up there. Still had to try to stick the landing a dozen times before Talion managed to get into that cave.

There's an ability uo can get where you use a sort of ground slam with your bow from the air by pressing X like a shadow strike. Probably would've made that easier since you can basically use it to teleport to any visible area within 400(?) feet
