Steam Controller stops working between games; have to restart Steam every time
Preamble: This is a really tough issue to even describe, so I don't know if others are having the same issue. But it's driving me absolutely insane.
I've been using the Steam Controller for *years*; I use it for all types of games, and make my own bindings all the time. This particular issue is new as of a few weeks ago.

When I turn on my Steam Controller, there is a ~90% chance that it will default to some weird layout that puts system volume on the left joypad, alt-tab on the start button, system zoom on left bumper, and more. Essentially, the layout is frustrating nonsense and totally unusable for anything (for me, anyway). I checked the default desktop layout and that isn't it. This behavior seems to be unrelated to any settings that I can find.
The real problem is: this is the layout that STAYS when I load up a game. Doesn't matter if I'm in Big Picture Mode, doesn't matter what game. Steam controller is stuck with this layout and, effectively, totally broken.

Turning it off and on again doesn't work. Alt-tabbing around/shifting Windows focus doesn't work. It's just stuck in this messed up and unusable layout. The only thing that works is restarting Steam entirely. And then the issue manifests again between game launches. Basically, my workaround at this time is to exit Steam every time I quit a game, and launch Steam fresh when I want to play. Extremely frustrating situation.

Anyone else running into this?

That's the Guide Button Chord Layout. Try tapping the Guide button to get you out of the layout.

Thought I was losing my mind; these controllers are on borrowed time and I want to hold on as long as I can!
