Dropped inputs for DS4/DS5 (wireless and wired) and random disconnects.
All buttons, both in game and in the controller settings menu except the sticks drop inputs regularly. Games having a high skill ceiling or games with mashing will receive about 50-60% of the inputs.

-Double taps are registered as a single tap or a button hold, not always but at an inconsistent rate. (This has also been tested in the controller test menu)
(The DS4 being used has been tested in the windows property menu, it recognises all inputs pressed with no issue, it's just steam)
-Inputs have also had a queuing issue, while most failed inputs are just dropped it sometimes will trigger the button pressed about half a second to a second later.
-DS4 controllers that are wireless also randomly disconnected from steam, the last input is repeated every frame until the controller reconnects or keys are pressed on the keyboard. In some games this hang causes the game to crash from the input spam. (RPCS3, the PS4 and other software outside of steam do not have this issue, so it's not a Bluetooth or controller issue)

This is a theory based on hundreds of in game disconnects, but it seems to be that the controllers battery power is separated into 25% segments, once it reaches one of these 25% thresholds steam for some reason disconnects it, a controller with 100% battery power will experience 3 disconnects before draining, now that my controller only holds 70% charge due to age, it disconnects twice before emptying.

3 years ago a reddit post was made on these issues, I have experienced them for around the last 5-6 years of playing (This is also a DS4 that was out of the box with a new ps4 all the way until now, 7 years of age), they have not been fixed and I assume most people just put up with it like I did, so no one (to my knowledge) has reported it yet. DS3 does not experience these issues either.
