Middle or index finger on trigger?
I personally use my index fingers for both triggers and shoulder buttons. Holding the controller like this I have no issues with grip or positioning (aside from the buttons being tiny). This seems to be how it is held in the trailer as well.
Trying to hold it with my middle fingers on the triggers is an uncomfortable mess.

Could this be the source of so many people having issues with the grip, or am I just lucky to have "good" hands?

I wonder the same currently.

I feel index for both is indeed more natural to me, i just take a bit time to switch to shoulders buttons, i guess i'm too used to my 360 controller.

Index is the way to go, middle just feels so wrong

Depending on the length of your fingers and the position of the controller, you can use the second joint of your index finger to push the shoulder button, whilst the fingertip remains on the trigger. Probably not viable for people with smaller than average hands though.

I think that's the way they seems to have designed to be pressed like.
I never had this idea and i just tried and it work well.

I have damm small hands, not sure what's considered long fingers tho.
