In thise case i would suggest using activators with the activation type "Start press" and toggle function to hold the keys and an activator set to "End press" to release the buttons.
Like Cyborg11 eluded to, you will need to make an activator. Depending on how you want the button to act, there are many different options available to you. I will assume what you want is to have one single tap of a button to send the command 'Hold left shift, press mouse 1, release both'.
What this configuration will do is it will hold the left shift for a tiny fraction of a second before sending the click command. It will then wait a small amount of time before releasing left shift which may or may not be necessary. This SHOULD give you that single press 'Hold left shift, press mouse 1, release both' that you are looking for.
Unfortunately, every single game is different and this may or may not work so you may have to play around with the delays to find something the game likes and will accept. The pickiest game I have found with exact timing is Overlord. Even then, I was able to do something very similar there and actually played through the entire game using it. You can bind literally anything to this kind of function, thanks to activators. It doesn't even have to be a physical button like in this demonstration I made for Overlord. Just keep in mind that some games are very lenient and just don't care as long as the two buttons are pressed at some point at the same time and others are so specific in what they allow it becomes a hunt and peck to see if you can find the exact correct settings it will like.
