Flying Sharks/Fish and Teleportation Glitch?

I'm not sure if anyone else has experienced this before, but I'm having this happen to me quite frequently. It's actually getting worse and worse for me, rendering this game almost impossible to play. Here's an example of what happens: I'll be running around, hitting trees with my hatchet and collecting materials. Then, I'll notice that I'm not getting notified that I'm collecting anything for a little while. I'll move around and try a different tree, then about 5 minutes later, I'll be teleported to a different location, around where I was previously. This also happens when I'm running or riding a mount and when I pay attention to my stamina bar decreasing. Sometimes I'll be riding or running and I'll see my stamina bar freeze, then I'll be teleported back to where I was about 5 minutes ago. This is turning into an unplayable game with these glitches because of the fact that if I'm ever in danger of being attacked, my game can show me escaping a dangerous situation, then I'll be teleported to my death. I don't know if this is a game glitch, or if it's my computer not being able to handle it well or not. This is an off and on issue that happens to me when I'm playing with friends on their online server. I have a fairly decent connection to the server (ping's anywhere between 20-50). The best way to know when I'm about to teleport is when I start to see flying sharks and coel/other fish.

My computer specs:

GPU: GeForce GTX 560 Ti
CPU: AMD A8-3850 APU with Radeon(tm) HD Graphics
Memory: 16.00 GB Ram (15.99 GB usable)
Current Resolution: 1920 x 1080, 60Hz
OS: Windows 7 Home Premium

If there's a simple solution to this problem that I have not thought of yet, I would appreciate it if someone would let me know what that is!

What server are you playing on? Could be a server issue.

It's not an official server, it's a private one from a friend who's local. He keeps it up to date and it runs smoothy most of the time, I think it's on the client-side.
